i-765 form

Nice greet again to my reader. I apologize for writing this in English because I am sitting in the university's computer lab with no Mandarin service. Well, I would assume my readers are US graduates and you would understand at least basic English. Today, I will let you know how to fill out the i-765 form, an application form for filing the OPT visa. 

Where can I get the i-765 form?

Please go to the USCIS website and search for i-765(click here). In the main site, you will the first hyperlink mentioned "Download form i-765". Click the link you will get the i-765 form in pdf format. 

How to fill the i-765 form? 

Filling the i-765 form is pretty simple, they are all basic information about  yourself except for question #9 to #17. In this section I will let you know how to fill in the information and if you still have any questions after reading this, please don't hesitate to send me an email. I would do what I can to help you. 

1. Question #9: Fill in your **SSN if you have one and leave it blank if you don't. 
2. Question #10: (Alien registration number or **i-94 card) There is a group of number on your i-94 card. TSA member stapled it on your passport when you last entered the border. Please check it out and fill it in CAREFULLY (pay attention to the last two digits, there is a space between the last two digits). 
3. Question #11: If you never registered for CPT or OPT, please tick NO in the box. If you do, fill in the information correctly. 
4. Question #12: The date of last entry to into US can be found in your passport or i-20. The immigration stamp would show the date of entry. 
5. Question #13: The city where you last entered the border. For example, Chicago, Los Angeles or New York. 
6. Question #14: Manner of last entry means the immigration status you last entered the US. In most cases, fill in "F1-Student" if you are a student. 
7. Question #15: If you are a fresh graduate who is seeking for OPT, please fill in "F1-Student" as well. 
8. Question #16: For most students, if you are not seeking for OPT extension of this is the first time you apply for OPT, fill in (c) (3) (b). 
9. Question #17: ELIGIBLE FOR OPT EXTENSION APPLICANTS AFTER 12 MONTHS.

**SSN: Full-term Social Security Number. If you have been participated in any kind of paid jobs, you will be issued a SSN from SSA office.
